What to Bring

• ORIGINAL passport or national ID, Iqama
• Three (3) photocopies of the passport (biodata page) or national ID, Iqama
• One (1) recent photograph for each applicant is required.
• Application number/ transaction reference number TRN/HAP ID provided by the visa office. For clients who will undergo the medical exam prior lodgment of papers to visa office, you have to complete “MY HEALTH DECLARATION” in Australian Immigration website to generate the HAP ID
• Eyeglasses or contact lenses, if worn
• Any medical reports or test results that you may have as a result of previous or existing medical condition
• Any medications you are taking
